Introduction:<\/strong> Pediatric forearm fractures are among the most frequent injuries encountered by orthopaedic surgeons. While traditionally managed with closed reduction and casting, surgical intervention with Elastic Stable Intramedullary Nailing (ESIN) has become the gold standard for fractures where stability or rotational alignment is at risk. The radius and ulna function as a unique rotational unit, biomechanically akin to intra-articular fractures, making near-anatomical reduction essential for restoring pronation and supination.<\/p>\n

Aims & Objectives:<\/strong> This presentation aims to provide a comprehensive roadmap for the pediatric orthopaedic surgeon. I will detail the fundamental principles of ESIN\u2014specifically the “oval concept” and three-point fixation\u2014while sharing practical “pearls” to optimise outcomes and “pitfalls” to avoid common complications. Furthermore, I will discuss the expanded utility of ESIN in complex patterns, including Distal Radius Metadiaphyseal Fractures (DRMDF), Monteggia Fracture Dislocations (MFD), and Radial Neck Fractures (RNF).<\/p>\n

Results & Clinical Outcomes:<\/strong> ESIN offers significant biological advantages by preserving the fracture hematoma and periosteum. When executed with precision, these techniques facilitate early mobilisation, high rates of union, and excellent functional recovery, allowing children to return to regular activities sooner than traditional methods.<\/p>\n

Conclusion:<\/strong> ESIN is a transformative tool that extends far beyond simple shaft fractures. By mastering modified techniques, surgeons can confidently manage complex forearm injuries while minimising risks of malunion or physeal injury.
