Although recent therapeutic advances have modestly improved outcomes, treatment options remain limited and the prognosis for patients with brain metastases is poor, underscoring an urgent need for novel therapeutic strategies. The blood-brain barrier (BBB) has long been recognised as a formidable obstacle, significantly hindering the delivery of effective treatments to the central nervous system. Recent evidence indicates that antibody-drug conjugates (ADCs) demonstrate superior efficacy against brain metastases compared to conventional cytotoxic agents. The introduction of next-generation ADCs holds the potential to further improve outcomes for this patient population. Strategically, a key therapeutic axis may lie not merely in circumventing the BBB, but in actively targeting and exploiting its unique biological properties. A detailed analysis of the mechanisms underlying BBB disruption specific to TNBC could enable the creation of novel, multi-faceted therapeutic frameworks. The development of robust preclinical models is also crucial to this effort. We provide a translational roadmap to address these unmet clinical needs and discuss an integrated therapeutic strategy for managing brain metastases in TNBC, aiming to transform the treatment paradigm for this challenging disease.<\/p>\n<p><strong>Biography :\u00a0<\/strong><\/p>\n<p>Dr. Masakazu Toi is a Japanese breast cancer specialist, surgeon, and researcher. He graduated from Hiroshima University School of Medicine in 1982 and completed his PhD in 1988. He also conducted research at the University of Oxford.<\/p>\n<p>He served as Professor of Breast Surgery at Kyoto University and has contributed extensively to breast cancer research, particularly in cancer biology, angiogenesis, biomarkers, and new treatments. Since 2023, he has been Director of the Tokyo Metropolitan Cancer and Infectious Diseases Center, Komagome Hospital.
